- Your business category (Production, Distribution, Theater, or Related Business)
- A short biography (max. 500 words, Word document)
- A statement of purpose (max. 500 words, Word document)
- Signed Code of Conduct Declaration
- Completed Board Skills Matrix (confidential; shared with Governance Committee post-election)
- (Optional) A one-minute introduction video
